Responsibilities Palm Point is growing! A fantastic opportunity has opened for a Licensed Social Worker, Licensed Mental Health Counselor, or Licensed Marriage Family Therapist to work in our Outpatient Program. The Social Worker /Therapist is a member of the multidisciplinary treatment team. Major job functions include but are not limited to: Identification, assessment and treatment of mental health conditions, such as depression and anxiety; case management/care coordination, particularly for individuals with chronic and/or complex medical conditions; patient navigation, especially for patients moving among different health care levels (e.g., inpatient, outpatient, home health, or long-term care); identification and referral for specialized services, such as drug and alcohol treatment, legal services, financial and employment counseling, and housing support; education and support programming (e.g., diabetes education, parenting classes, domestic violence support programs) for individual and groups; assistance with entitlements, medications, transportation, and advance directives; assessment and intervention in domestic violence and child abuse situations; Outreach and coordination with other community resources and agencies; conducts psychosocial assessment and information related to diagnosis/treatment experiences, age, developmental stage, life style, traits/disorders and psychopathology; provides clinically sound individual, family, and group therapies within required time frames and establishes therapeutic relationships with patients and families; completes documentation per policy and procedure; provides education/consultation to client/family, treatment team and community; communicates psychosocial impact of diagnosis, prognosis, and treatment interventions; ensures good clinical judgment to ensure optimum patient care and safe discharge planning; participates in continuing education activities; uses de-escalation, mediation, conflict resolution and other less-restrictive responses in relationship to the level of the patient while safely managing patient aggression; complies with standards, policies/procedures, laws and regulations; assists in developing and maintaining community resource information; performs marketing functions as assigned; assists in the maintenance of the program environment in compliance with policies of safety and security. Qualifications Required Qualifications Master's degree in Social Work, Mental Helath Counseling,or Marriage and Family Therapy Current valid social work license, e.g. MSW, LCSW, LMSW Minimum 2 years of experience in identifying and accessing resources to meet basic needs and in securing community-based assistance. Proficiency with Microsoft Office Programs, e.g. Word, Excel, PowerPoint, Access Preferred Qualifications 3-5 years of experience working as a licensed Social Worker Experience working with children, adults, and aging population preferred Knowledge of community health and social service agencies and additional community resources
