JOB SUMMARY
Florida Blue Medicare is a technology leveraged health plan. However, we will never abandon our commitment to human contact and service. We embrace meeting members where they are and that may not be electronically. To meet that commitment, Florida Blue Medicare has developed a Field Based Community Health Program (CHP). The purpose of the CHP is to provide member and family support via an in-person home visits, delivered with compassion and empathy focusing on the member's social needs. The CHW program will ensure that Florida Blue Medicare members have the tools and resources needed in their local community in order to live healthier lives.

ESSENTIAL FUNCTIONS

  • The essential functions listed represent the major duties of this role, additional duties may be assigned.
  • Conduct home visits with Florida Blue Medicare members with chronic conditions and/or barriers to care, to reinforce following the care plan, adjusting tasks to fit with the members culture, language, and religion, improve access to care, identify health care needs, and reduce unnecessary hospitalizations. (40%)
  • Educate healthcare professionals and providers on opportunities for improving and understanding the social determinants of health that may be impacting members health and treatment plan through various communication channels. (5%)
  • Collaborate and coordinate with the Case Management, Health Management and Re-admission Prevention teams, to identify and resolve member barriers to self-management. (10%)
  • Assist with care plan interventions and member education during in-home visits, help develop care management strategies, and work with team members to provide linkages for the various health and social needs of members. (10%)
  • Assist members to identify socio-economic issues that affect their overall health and develop health/social management plans and goals.. (10%)
  • Identify gaps and opportunities to strengthen systems of care within the members community and assists members in utilizing community services, including scheduling appointments with social services agencies, (including transportation vendors), and assisting with completion of applications for eligible programs. (5%)
  • Assist with patient medication adherence by identifying knowledge gaps and referring to internal and external pharmacy resources. (5%)
  • Educate on self-management strategies (i.e. nutrition, symptom tracking and reporting). As the member advocate, accompany members to appointments as needed. (5%)
  • Enter and maintain member records in electronic health record system, compile reports and complete other program documentation in a timely manner (e.g. progress notes, letters), and other administrative responsibilities as needed. (10%)

    REQUIRED WORK EXPERIENCE
    2+ years related work experience. Experience Details: 1-2 years experience providing clinical services; experience in community/outpatient setting preferred.

    REQUIRED EDUCATION
    High school diploma or GED

    Required Licenses and Certifications
    CCHW - Certified Community Health Worker 1 1/2 Yrs

    ADDITIONAL REQUIRED QUALIFICATIONS
    1-2 years experience providing clinical services; experience in community/outpatient setting preferred. Community Health Worker Certification or equivalent required experience for certification within 18 months
    Experience using a computer including the use of Excel and Electronic Health Records.
    Meet all requirements set forth by local state i.e. first aid, CPR, TB screen, etc.
    Must have a valid driver's license, automobile insurance and reliable transportation

    ADDITIONAL PREFERRED QUALIFICATIONS
    Community Health Worker Certification

    Requires flexibility to work in situations in which the working conditions are unknown, as environments and situation of patient homes may vary.

    Ability to communicate medical information to health care professionals and care coordinators over the telephone.

    Knowledge of formal and informal community agencies and resources. Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services.

    Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.

    GENERAL PHYSICAL DEMANDS
    Sedentary work: Exerting up to 10 pounds of force occasionally to move objects. Jobs are sedentary if traversing activities are required only occasionally.

    PHYSICAL/ENVIRONMENTAL ACTIVITIES
    Must be able to travel to multiple locations for work (i.e. travel to attend meetings, events, conferences). Frequently May be exposed to outdoor weather conditions of cold, heat, wet, and humidity. Occasionally
